Nurse Coaches join in a coaching and discovery process with clients to increase awareness of desired healthy lifestyle goals, life balance/satisfaction, behavioral change, and much more. These Nurse Coach and client guiding principles bring clarity to this dynamic coaching relationship/partnership.
Nurse Coach
- I will listen with full presence to the client’s concerns and hopes in a safe, supportive environment (e.g, in person, at the office, or via Skype) to increase trust and self-exploration of health and wellness goals.
- I will respect the client as the authority for her/his own health and wellbeing.
- I will support the client’s inner wisdom, intuition, and innate ability to determine the next steps in her/his process for best possible outcomes.
- I will involve the client in formulating SMART goals (Specific, Measurable, Achievable, Realistic, Time-lined) in her/his action plan.
- I will obtain the client’s consent to coach in vulnerable areas and will maintain confidentiality.
- With permission, I will offer guidance and health education for specific health concerns and/or conditions (acute/chronic), nutrition, exercise, stress management, and other areas related to overall lifestyle health and wellbeing.
- I will recommend other healthcare professionals when I feel issues are outside my scope of practice and experience.
- I will be punctual and keep scheduled appointments.
Client

Services
Coaching is a relationship of respect between the Nurse Coach and client/patient. As a Nurse Coach, I have a rich background and experience in lifestyle health and wellbeing. I will recognize your unique story, beliefs, values, history, and culture. Together, through the coaching and change process we will explore your various challenges, obstacles, and successes. (Note: Coaching is for individuals who are emotionally and psychologically healthy and is in no way considered or construed as any type of therapy. If needed, the Nurse Coach will refer the client to another qualified professional.)
Coaching Approach
Coaching is a collaborative partnership where trust is established. My coaching approach is integrative, integral, holistic, and skilled. This means that I see you as a whole person from many perspectives. Together, we will identify the topics and concerns that you want to work on towards change and those areas that arouse energy and excitement.
We will focus on your strengths and values to move you towards healthier lifestyle behaviors, and how you can sustain and integrate new patterns, strategies, goals, and action plans. Throughout our coaching sessions, I will share many strategies that may be new for you, such as awareness practices, relaxation, and imagery, to enhance your success.
The Optimal Coaching Experience
This coaching includes 1 dedicated intake session and then 9 additional coaching sessions over a 3 to 4 month time period. These calls are self-scheduled using an online scheduling system provided to the client. These calls are 60 minutes in duration.
The coaching session will be conducted online (Google Meet or Zoom) or on the phone at an agreed- upon day and time. If the coaching session is by phone, you (client) will agree to call me (Nurse Coach) at the agreed-upon time and phone number.
We both agree to start and finish on time. If you are more than 10 minutes late for the coaching session, I will assume you have canceled the session, and I will still need to bill you for the session.
In between regularly appointed coaching sessions, I will use email "post-session recaps" to enhance and anchor the benefits of your coaching. This will create a "journaling" of your success.
Between coaching sessions, you may have an urgent question, request for feedback or want a confidential response to some situation where you don't want to wait for your next session. This is a "just in time coaching" where you can use the ‘My Logs’ section of the online private portal to email me and get a timely response. Please allow 24-48 hours for a response.
Cancellation
If the coaching session needs to be canceled or rescheduled, please allow at least 24 hours in advance of the scheduled session to notify me. If this is not done, you will be billed for the session. If I need to reschedule with you, I will give you a 24-hour notice.
Private Client Access
Everything we will be using in this coaching experience (tools, forms, schedules, documents, resources, etc) will all be housed conveniently on a secured and confidential private client online portal. Upon enrolling for the coaching experience, you will be given a unique username and password. At your convenience you may log into this private client portal area to access, review and/or update the tools, documents, resources, etc that we'll be using throughout our coaching together. I will provide you with the access information and further instructions once we begin coaching.
Confidentiality
I your nurse coach, represents, warrants and agrees as follows:
All information provided by the Client to the Nurse Coach during the course of the coaching relationship will be deemed "Confidential Information" and kept strictly confidential unless otherwise agreed in writing or through verbal permission. The Nurse Coach will not disclose any communications, either verbal or written, that have taken place either between the Nurse Coach and the Client or with regard to this Agreement without the prior written consent of the Client.
This confidentiality restriction also applies to testimonials, which the Nurse Coach may use for promotional reasons only with the consent of the Client. The Client may request anonymity in connection with any testimonials to be used.
Notwithstanding the foregoing, "Confidential Information" does not include information that is:
(a) previously known to the Nurse Coach prior to discussions regarding this Agreement, free from any obligation to keep it confidential, or
(b) publicly disclosed by the Client either prior to or subsequent to the receipt by the Nurse Coach of such information, or
(c) independently developed by the Nurse Coach without any access to Confidential Information, or
(d) rightfully obtained by the Nurse Coach from a third party lawfully in possession of Confidential Information who is not bound by confidentiality obligations to the Client.
The Nurse Coach may disclose Confidential Information if required to do so under applicable law, rule or order; provided that the Nurse Coach, where reasonably practicable and to the extent legally permissible, provides the Client with prior written notice of the required disclosure. As a Registered Nurse, I am legally and ethically required to report any suspected or known abuse or neglect of vulnerable populations including children, elderly, dependent adults and people with disabilities.
Payment
You have two choices as to how you pay for your coaching services:
_____Payment Option A: Pay In Full
With this option, the total amount of the agreement ($1600) is due at the beginning of our coaching agreement. This payment is due prior to our first coaching session.
_____Payment Option B: Pay Each Month
With this option the agreed upon monthly payment will be made by credit card and automatically processed every 30 days. The first payment of $400 will be due prior to our first scheduled session. The total monthly payment will be $400/month for an additional 3 months.
Conflict of Interest
If a conflict of interest arises, you and I will work together and discuss and take steps to resolve it respectfully and objectively. If this cannot be accomplished, the coaching agreement will be terminated and a referral to another professional will be made.

If after participating fully, you do not agree that the process of coaching is worth every penny of the price you paid, just let me know anytime within the first 30 days of your coaching experience and I will promptly make arrangements to refund your charge card for the entire amount.
